Evaluer l'importance de l'IRM endorectale et du PET/CT au 11C-acétate ou à la 18F-choline chez les patients avec un cancer de la prostate en rémission >8 ans après radiothérapie (RT)(PSA =
0.5ng/ml)
Le PET avec les traceurs 11C-acétate, 11C-choline et 18F-fluorocholine a montré des résultats prometteurs dans la détection des rechutes du cancer de la prostate mais la distinction entre des lésions bénignes (prostatite chronique, fibrose, hyperplasia) et une récidive locale au niveau de la prostate reste difficile. L'interprétation de l'IRM endorectale post RT dans le cancer de la prostate reste également malaisée, avec la difficulté pour différencier une fibrose post-RT d'une récidive tumorale. Pour mieux évaluer le rôle du PET/CT au 11C-acétate et/ou à la 18F-choline pour les patients présentant une récidive biochimique d'un cancer de la prostate après radiothérapie (± déprivation androgénique), il est d'abord nécessaire d'établir une référence d'imagerie chez des patients guéris (patients en rémission clinique et biochimique -PSA<0.5 ng/ml- 8 ans ou plus après RT) et traités de la même manière. Cette étude a donc par but de définir un référent d'imagerie pour un cancer guéri au niveau de la prostate. L'interprétation de l'IRM endorectale et du PET/CT avec les traceurs 11C-acétate et 18F-choline post RT dans le cancer de la prostate reste malaisée, avec la difficulté pour différencier une fibrose post-RT d'une récidive tumorale. C¿est pour ces raisons que l'objectif de cette étude est d'investiguer par IRM endorectale et PET/CT au 11C-acétate et/ou à la 18F-choline post RT les patients présentant un cancer de la prostate en rémission. Chez ces patients on peut s'attendre à étudier des images « normales » après une RT. La connaissance des images « normales » après une RT est importante pour différencier les images bénignes des images d'une rechute locale après la RT. Evaluation des avantages potentiels de la technologie PET-CT par rapport à la technologie CT conventionnelle dans la simulation et la planification des traitements de radiothérapie (RT) en 3D Pour planifier tout traitement anti-cancéreux, une définition exacte de l'extension de la maladie ('staging') est nécessaire pour confirmer ou infirmer la présence de métastases à distance, ou d'adénopathies loco-régionales cliniquement non évidentes. Le but du projet est de développer l'utilisation du PET-CT pour la simulation et la planification des traitements de radiothérapie (RT) en 3D, et d'évaluer les avantages potentiels de cette nouvelle technologie par rapport au CT conventionnel utilisé actuellement. Nous prévoyons d'effectuer des PET-CT chez 100 patients atteints des différents types de cancer dans le Service de Médecine Nucléaire pour planifier la RT (simulation virtuelle en 3D directe sur les images du PET-CT). Dans tous les cas les patients ne seront sélectionnés que si l'étude PET-CT est indiquée pour compléter le bilan diagnostique de leur maladie. Notre projet sera un des pionniers à étudier la faisabilité de l'utilisation directe de la technologie PET-CT dans le domaine de la simulation virtuelle en RT-3D. Cette étude va permettre en plus de développer un programme de recherche clinique transversale intra-départementale entre les services de Médecine Nucléaire et de la Radio-oncologie des HUG. Le drainage plus important est celui qui se fait par les ganglions iliaques internes, suivi par celui des ganglions obturateurs et présacrés, puis par les ganglions situés dans la région des vaisseaux pudendaux internes.
