MRSA
Nature du problème
Depuis plusieurs années, des infections causées par des souches de staphylocoques dorés résistants à la méthicilline (MRSA) et souvent multi-résistants ont été signalées partout dans le monde. La prévalence de l'infection varie entre 0 et 50 %; elle est plus élevée dans les hôpitaux universitaires, en Suisse comme ailleurs. Situation aux HUG MRSA est un problème aux HUG depuis le début des années 90, en proportion épidémique depuis 1993-1994. Des efforts de contrôle ont été instaurés, avec succès. Malheureusement, nous assistons depuis le milieu de l'année 1999 à une nouvelle augmentation du nombre de cas, qui prend des proportions inquiétantes depuis 2000. Cette épidémie touche tous les secteurs del'institution, à des degrés variables. L'augmentation de l'incidence est attribuée en partie à l'augmentation du réservoir épidémique de provenance extérieure aux HUG. Pourquoi ce problème nous inquiète-t-il ? Les infections à MRSA sont grevées d'un taux important de morbidité et de mortalité. Elles viennent s'ajouter aux autres infections nosocomiales; leur traitement impose le recours à des antibiotiques relativement toxiques, pourvus d'effets secondaires et nécessitant des examens complémentaires pour assurer leur bonne utilisation. Enfin, la transmission intra-hospitalière est le reflet d'un manque de qualité des soins. Prévention Les principales mesures de lutte contre la propagation de l'infection sont l'hygiène des mains, l'identification des patients colonisés ou infectés, les mesures d'isolement destinées à protéger le milieu hospitalier, les précautions lors de transferts, et la réduction du portage. Projets de recherche clinique 1) Identification du réservoir Les objectifs sont : a) recueillir des informations épidémiologiques permettant une lutte globale et une action sur tous les réservoirs de l'épidémie ; b) accélérer la prise en charge et permettre l'orientation optimale des patients porteurs dès l'admission, ainsi que la détection des cas d'entourage ; c) optimaliser l''occupation des chambres d'isolement en ne les utilisant que pour des cas confirmés. 2) Prévention des infections à MRSA aux soins intensifs et mise en place d'un test ultra-rapide Les objectifs sont : a) accélérer la prise en charge et permettre l'isolement précoce de patients colonisés; b) diminuer les taux de transmission croisée et d'infections graves à MRSA. 3) Evaluation retrospective de l'épidémie à MRSA aux HUG L'objectif principal est de mieux comprendre les facteurs qui ont favorisé l'augmentation du nombre de cas MRSA depuis 2000 avec analyse épidémiologique et moléculaire de différentes souches MRSA isolées aux HUG.
