primary and metastatic liver tumor »
Il s'agit d'un projet de traitement interventionnel minimalement invasif dans le réseau multi-institutionnel CO-ME. Le but est de développer et valider, avec plusieurs partenaires de l'industrie et d'autres Universités, une nouvelle méthode pour le traitement minimalement invasif de tumeurs, notamment la thermothérapie percutanée des tumeurs hépatiques par radiofréquence sous guidage par résonance magnétique (IRM). Plusieurs partenaires de l'industrie contribuent à la réalisation de ce projet par la mise à disposition gratuite d'équipements, de logiciels, de spécialistes, etc. br> Le rôle de la radiologie des HUG est la conception, la coordination et l'intégration des différentes composantes et la validation clinique de l'ensemble de la méthode. Aujourd'hui, un total de CHF 2'346'200.- de fonds de recherche ont été attribués au département de radiologie des HUG par différentes sources, dont CHF 1'831'200.- par CO-ME dans deux phases du projet, 2002-2005, et 2005-2008. Ces ressources nous permettent le financement de postes de physiciens de recherche. Dans la première phase, nous avons validé la faisabilité du traitement de tumeurs hépatiques par radiofréquence sous guidage interactive par IRM dans un système à champ ouvert de 0.23T (cf. publication). La deuxième phase sera réalisée avec un nouveau système d'IRM ouvert de 1.5T. Cette augmentation en puissance de champ magnétique nous permet de valider une méthode de thermométrie in vivo afin de monitoriser et contrôler la déposition de l'énergie thermique « on ¿line ». Les étapes sont les suivantes: 1) passage à un nouveau système de radiofréquence multipolaire IRM-compatible (réalisé); 2) validation du logiciel de thermométrie par corrélation anatomo-pathologique de lésions thermiques expérimentales induites chez le cochon (réalisée); 3) Mise en fonction du nouveau système IRM ouvert; 4) intégration d'un système d'ultrasons à haute résolution à l'intérieur du champ magnétique; 5) adaptation du logiciel thermométrique et du générateur de radiofréquence multipolaire au nouveau système IRM; 6) développement des commandes interventionnelles dans le champ magnétique; 7) développement de séquences d'imagerie « fluoroscopiques » et avec compensation respiratoire pour l'imagerie; 8) validation d'un algorithme de simulation de la thermothérapie par ordinateur avant le geste et 9) application clinique des différentes étapes avec un bénéfice direct pour les patients.
