Amnésie

Ce projet combine l'évaluation clinique et neuropsychologique et l'évaluation par potentiels évoqués et cartographie cérébrale pour explorer les mécanismes de l'amnésie (perte de mémoire) et comment le cerveau humain sain adapte la pensée à la réalité en cours. Le phénomène clinique à la base de cette recherche est la confabulation spontanée, une confusion profonde de la réalité dans la pensée. Les patients agissent sur la base de mémoires qui ont guidé leurs actions dans le passé mais qui n'ont pas de lien avec le présent. Nos résultats récents :
(1) Adaptation de la pensée à la réalité : Nous avons découvert un mécanisme dépendant de la partie postérieure du cortex orbitofrontal qui est cruciale pour l'adaptation de la pensée à la réalité en cours. Il paraît que cette région adapte le format cortical des mémoires selon leur rapport avec la réalité. Cette adaptation se fait avant qu'un sujet se rende compte du contenu précis d'une mémoire. Notre hypothèse est que cette adaptation de la pensée dépend d'une capacité intacte à l'extinction - la capacité d'apprendre qu'une anticipation auparavant correcte ne l'est plus. Nous explorons actuellement le lien entre le système de récompense et l'adaptation de la pensée à la réalité et le rôle du système dopaminergique. (2) Mécanismes de l'amnésie : en utilisant la cartographie cérébrale, nous avons découvert chez un patient amnésique qu'il était incapable d'activer normalement des réseaux neuronaux distribués. Son traitement restait essentiellement visuel. Ce résultat a récemment été confirmé chez un group d'amnésiques.

D'autres sujets de recherche prioritaires du service de neurorééducation sont les suivants (voir pages web correspondantes) :

Traitement assisté par ordinateur de l'anomie dans l'aphasie et les mécanismes de récupération d'une aphasie (directeur du programme : Dr M. Laganaro). Nous avons trouvé un effet spécifique aux items entraînés et explorons actuellement les limites (intensité) et la spécificité de cette modalité de traitement.
Facteurs qui influencent la conscience de l'espace (wareness) et l'exploration dans l'héminégligence spatiale ainsi que la rééducation de celle-ci (directeur du programme : Dr R. Ptak).
